Everyone is different, with different needs. No two people are the same.
In the past, community services provided to clients were very rigid, where people were categorised according to their condition, and not their individual needs.
We know now that this is not an appropriate way in which to offer support. Every person is unique in their own way.
For this reason, it is necessary that there are several flexible models of support available to assist clients to fulfil their needs. Each model should be applied individually to cater to the specific needs of the client and should be developed and applied by all key stakeholders in the client’s life.
Examples of different models of support include:
• Active Support – Assisting people to engage in meaningful activity and relationships as active participants.
• Advocacy – Standing alongside an individual who is disadvantaged and speaking out on their behalf in a way that represents the best interests of that person
• Behaviour Management – Support strategies implemented to minimise the incidence of client behaviour, developed from thorough assessment of the person's needs and investigating the reasons behind behaviour occurring in the first place
• Case Management – A collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s holistic needs through communication and available resources to promote quality client outcomes.
• Community Development and Inclusion – Providing opportunities for people to be a valued member of the community (through activities, employment opportunities, socialising,etc)
• Day Services – Activities are organised or provided directly by community service organisations forclients/residents.
• Health Promotion – Enabling people to increase control over their own health through education and support, resulting in better health
• Home based support – The provision of practical assistance to help people to live as independently as possible in their own homes.
• Living Skills – Support and assistance with the everyday tasks of life
• Person Centred Planning - Putting people at the centre of service delivery
• Residential Services – Provide accommodation and support for people who need support in everyday life, for example, people who are frail or have a disability.
• Respite - The provision of short-term, temporary relief to those who are caring for others
• Supported Employment – Supporting people with disabilities to access employment opportunities and provides on-going support to maintain employmentThe aim of these models is to ensure that the client is participating as fully as possible in their own lives, maximising their independence.
