Changes in Client/Resident Health

On top of your identifying variations in personal care and aid requirements, you also have a responsibility to identify and report any variations or concerns you have regarding a client/resident’s health. Remember that in some cases, early detection is vital and will increase the person’s chance of recovery. Reports may be done verbally (particularly if it is urgent), however they must always be followed-up with written documentation.

Variations the client’s physical condition may include:

• Weight loss or weight gain

• The client reporting that they feel unwell

• Aches and/or pains

• Skin conditions

• Seizures

• Changes in skin tone or colour

• Oral health anomalies

• Nail status

• Tiredness

• Swelling of feet or ankles

• Frequent influenza

• Gastrointestinal problems

• Vision or hearing problems

• Urinary issues.

It is important that as soon as you are made aware of anything that varies from the client/resident’s Care Plan, or any health concerns, that you document this and report it immediately to your Supervisor or Manager for follow up. It is important that the person’s situation is addressed as soon as possible so that their Personal Care Plan can be adjusted with minimal impact on the older person.

Our aim is to maximise the older person’s quality of life, so early detection and reporting of any changes it a critical aspect of support when working in aged care.

Monitoring the Use of Aids & Equipment

Whilst it is the responsibility of your organisation and yourself to maximise the older person’s abilities and capacity through environmental modifications or the provision of adaptive equipment, there will be occasions where this is not possible or where such modifications and equipment may have a negative impact upon the older person. As an Aged Care Worker, you have a responsibility to identify any variations in the personal care support requirements and/or aid requirements of your clients, and report them accordingly. This enables the organisation to make the necessary arrangements to have the person reassessed and their equipment/aids altered to meet their changing needs and preferences. Keep in mind that the Care Plan is developed to meet the client’s needs, preferences and goals at the time of development, so they will need constant review as these things change over time.
